Sign up Form for Photo Shoot Days, April 8,& 9th

Please send the request below with at least 2 dates April 8th and/or April 9th,and times Between 2:30PM and 8:30PM you will be available to have your picture made. You are not obligated to purchase photographs, but please make an effort to make our pictoral directory complete! Please note. There will be a make-up day for photos on Good Friday, April 10th if needed. You will be notified by email or phone to confirm your appointment. Try to be at the Church, ready for pictures 30 minutes prior to your scheduled time.
